The following must be completed before you are able to serve in any program involved with children:
*Forward Clearance via email to info@forkscommunitychurch.org or give to Church Office Administrator to be put in file.
We can't wait to start serving with you!
The following must be completed before you are able to serve in any program involved with children:
- Sign a Disclosure Statement
- Obtain a PA State Police Criminal History Clearance*. Go to: https://epatch.pa.gov/TandCVolunteer
- Obtain a PA Department of Public Welfare Child Abuse Clearance*. Go to: https://www.compass.state.pa.us/cwis/public/home
- Obtain an FBI Clearance* (if in Pennsylvania for less than 10 years). See Church Office Administrator or Ministry Lead for instructions/code.
- Training Session(s) with Ministry Lead
